OSAP Information Mismatch (Name & Date of Birth)

If you want to make a change to any of these items, you must provide a letter to your financial aid office identifying the reasons for the change and the correct information. You may also be required to provide documents to support the change.

If you mistakenly entered your name incorrectly on your OSAP profile, you must submit a letter along with one of the following documents to correct the error:

  • SIN Card or temporary SIN Card 
  • SIN Confirmation letter from Service Canada that contains your SIN  
  • Change of Name Certificate issued by the Registrar General
  • Marriage certificate showing your new name.

If you mistakenly entered your date of birth incorrectly on your OSAP profile, you must submit a letter along with one of the following documents to correct the error:

  • Birth Certificate 
  • The front and back of your Permanent Resident Card

  • Your Record of Landing

  • Your Confirmation of Permanent Residence

  • Your Protected Persons Status Document (issued prior to January 1, 2013)

  • Your Refugee Travel Document
